Integrative Healthcare Associates is working to get accredited by the NCQA to become a Patient-Centered Medical Home (PCMH). A PCMH is a model of healthcare delivery that aims to provide comprehensive, coordinated, and patient-centered care. In a PCMH, a primary care provider leads a team of healthcare professionals to provide continuous and coordinated care for patients, with a focus on prevention, wellness, and chronic disease management. Key features of a PCMH include enhanced access to care, care coordination, patient engagement, and quality improvement. The goal is to improve the patient experience, enhance health outcomes, and reduce healthcare costs through better coordination and management of care.

IHA survey results 2024
o 93% of patients felt our providers spent enough time answering their questions.
o 83% of patients felt they were able to get a well visit (physical exam), as soon as they needed.
o 90% of patients felt they were able to successfully schedule a same day visit when necessary.
o 88% of patients felt that lab or imaging test results were communicated with them in a timely manner.
o 100% needs were met regarding controlling their diagnosis of primary hypertension.
